This form is available for PDC Staff to access in Meridian Forms. The form MUST BE PRINTED ON YELLOW PAPER then e-mailed to the Parking Office.

Not neccessary to save this document. Will be given to the Department of Police and Public Safety.


Detailed Workflow

The Contractor parking pass request needs to be e-mailed to at least 5 days ahead of time. If it is less than a 5 day notice, still send them an e-mail and they will try to accommodate the request. They will e-mail the parking pass when it is ready to be picked up by the contractor.
